Report surplus lines insurance transactions to The Surplus Line Association of Arizona ("SLA-AZ") by or before the due date.

  • Go to the SLA-AZ Home page (www.sla-az.org) and click the "File Online Now" button in the upper right corner of the page.

  • In the upper right corner of the web page, log in with your username and password. If you do not already have an established username and password, click on the "REGISTER" button and follow the steps to become registered with the SLA-AZ.
  • Enter the required information for each transaction. You must report every transaction, including transactions that have a zero dollar amount (such as a change to the primary insured, change of policy number, etc.). A change to the insured's street address or phone number is not a surplus lines transaction and should not be reported.
  • The SLA-AZ system will send a stamping fee invoice to the e-mail address you provided when you registered with the SLA-AZ. You must pay stamping fees directly to the SLA-AZ. DO NOT REMIT STAMPING FEES TO THE DEPARTMENT OF INSURANCE.

Download and save your "Transaction Report" spreadsheet.

  • Click the Reports tab
  • Select the report period
  • Click the "Run Transaction Report" button. The system will display your transactions for the selected period in the lower half of the screen. If you had no transaction effective during the period, the system will display "No filings." Do not file a report with the Department of Insurance if you had no transaction that was effective during the period.
  • If you had transactions effective during the period, click the "Save to Excel button. The system will ask if you want a "Short Form" or "Long Form." You may want to download the "Long Form" for your internal reconciliation purposes. You must include the "Short Form" as part of your tax filing.

Reconcile the information in the spreadsheet to your records to ensure that all of your
transactions effective during the period are reflected in the spreadsheet.

Submit any corrections that need to be made to the SLA-AZ system and repeat steps 2 through 4 as necessary until all transactions effective during the period are properly reflected on your report. If you have any questions about the data in your report or about how to make corrections to your records maintained in the SLA-AZ system, contact the SLA-AZ using the contact information on their web site (www.sla-az.org).

Produce your Transaction Summary and Tax Report.

  • In the "Report Period" dropdown box, select the appropriate period.
  • Click the "Run Tax Report" button. The text on the button will change.
  • Check the "Click to Download button. The system will display the Transaction Summary and Tax Report ("Form E-SSL" for Arizona-only transactions or "Form E-SL/M" for multi-state transactions).
  • Review the tax report.
    • Make sure the report is for the correct period (shown in the upper right portion of the report).
    • Ensure that the infromation on the report corresponds to information in your records. If it does not, contact the SLA-AZ using the contact information on their web site (www.sla-az.org).
  • Save the report to your computer or computer network.

Remit your tax report(s), transaction report(s) and tax payment. You can either

  • Submit your reports and payment electronically using the NAIC OPTins system (www.optins.org), OR
  • Deliver or mail your reports and payment to the following address:
